Formative research report: Understanding caregiver decision-making on routine immunization in zero-dose communities in Cameroon

This formative research study explores how caregivers in Nkolndongo, Cameroon make decisions about routine immunization for zero-dose and under-immunized children. Through 135 in-depth interviews analyzed using the WHO Behavioral and Social Drivers (BeSD) framework, the report reveals clear generational and gender differences in attitudes, social influences, trust in the health system, and practical barriers. Younger mothers show strong motivation but face logistical challenges, while older caregivers, especially fathers and grandfathers, express greater distrust and negative social norms. The report maps decision-making pathways for each caregiver group and offers targeted recommendations to strengthen vaccine confidence, improve service accessibility, and design culturally grounded, caregiver-specific social and behavior change interventions.
